I've heard of dentists that put their patients under if they dislike dental appointments. Maybe that is something that you could look into.

I have terrible dental anxiety also. It comes from a nasty childhood dentist. I had a really good dentist I loved before I left after getting married.
Now I found a new dentist that does sedation dentistry. I haven't tried it yet due to low funds and the amount of work I need done but it could work for you too!
